Your eyes hurt by 4 p.m. Here is what is actually going on

Sore, dry, heavy eyes after a day at a screen are not imagined and not permanent damage — but they are a signal. What causes computer vision syndrome, what genuinely helps, and when it is worth getting your eyes checked.

By the middle of the afternoon your eyes feel gritty. The text on the screen goes soft for a second before it sharpens again. There is a dull ache behind your eyebrows, and by evening it has turned into a headache. If you work at a computer in Kathmandu, this is one of the most common things we see — and one of the most fixable.

It has a name: digital eye strain, or computer vision syndrome. It is not a disease, and it does not mean your screen is damaging your eyes. It means the visual system has been asked to hold one demanding posture for hours without a break, and it is telling you so.

Why a screen is harder work than a book

Three things happen at once when you settle in front of a monitor, and they compound each other.

The first is blinking. At rest you blink somewhere around 15 to 20 times a minute. Concentrating on a screen, most people drop to roughly a third of that, and many of the blinks that remain are incomplete — the lid does not fully close, so the tear film is never properly resurfaced. That is the grittiness. It is a tear-film problem, not a "tired eye" problem, which is why resting your eyes helps less than you would expect and why the discomfort is worse in an air-conditioned room or under a fan.

The second is focusing effort. Holding focus at 50–60 cm keeps the focusing muscle inside the eye contracted for hours. Like any muscle held in one position, it starts to protest — and its first sign of fatigue is exactly what people describe: the screen drifting soft and then snapping back.

The third is eye teaming. Both eyes have to converge slightly and stay converged. If your eyes are even a little reluctant to hold that alignment — very common, and often never diagnosed — the effort of holding it is where the ache above the eyes comes from.

What actually helps

Break the focus, not just the screen

The 20-20-20 rule is well known and worth doing properly: every 20 minutes, look at something about 20 feet (6 metres) away for 20 seconds. The distance is the point. Standing up and checking your phone is not a break — it is the same focusing distance with a smaller target. Look out of a window, or down the length of the room.

Blink on purpose

A few times an hour, close your eyes gently and squeeze for two seconds. This is not the same as a rapid blink; the slow, complete closure is what spreads the oily layer that stops your tears evaporating. If your eyes are already dry, preservative-free artificial tears used three or four times a day are safe and help — and are far better than the redness-relief drops sold over the counter, which constrict blood vessels and rebound worse when you stop.

Fix the geometry of your desk

  • Put the top of the monitor at or just below eye level, so your gaze travels slightly downward. A downward gaze means less of the eye surface is exposed, and less tear evaporation.
  • Sit an arm’s length away — around 50 to 70 cm for a laptop or desktop.
  • Get the screen out of the direct line of a window. Glare makes you squint, and squinting is its own source of ache.
  • Raise the text size rather than leaning in. Leaning in shortens the working distance and increases the focusing load.
  • Point the fan away from your face. A moving airstream across the eyes will dry them faster than the screen ever will.

Check the prescription you are actually wearing

A surprising share of the strain we see comes down to spectacles that are right for distance and wrong for a desk. An uncorrected astigmatism of less than a dioptre, a reading addition that is a touch too strong, or progressive lenses whose intermediate zone sits at the wrong height for your monitor will all show up as afternoon fatigue rather than as blurred vision. This is the part you cannot troubleshoot at home.

When to get it looked at

Screen fatigue that lifts overnight and is better at weekends is ordinary and is worth managing with the habits above. Book an examination if any of the following is true:

  • The discomfort no longer settles overnight, or is present at the start of the day.
  • You are getting headaches most days, or the ache is concentrated around one eye.
  • Your vision is blurred at distance too, or blurs and does not clear.
  • You see double, even briefly.
  • Your eyes are persistently red, sticky, watering, or feel as if something is in them.
  • You have started avoiding reading because it is uncomfortable.

Double vision, a sudden change in vision, eye pain with nausea, or flashes and a shower of new floaters are different — those are reasons to be seen the same day, not next week.

What an examination for this involves

A proper assessment for screen-related symptoms goes beyond reading a chart. We check the refraction at your actual working distance, not just at six metres; we test how well the eyes team up and hold convergence; we look at the tear film and the lid margins under the slit lamp, because blocked oil glands are a common and treatable cause of evaporative dryness; and we examine the back of the eye, because a first eye test for an adult in their thirties or forties is also the first chance to pick up glaucoma or an early retinal change that has no symptoms at all.

Where the problem is eye teaming rather than focus, a short course of vision therapy — structured exercises, done at home and reviewed in clinic — often resolves what no lens change will. Where it is dry eye, treating the lid margins matters more than the drops.

Common questions

Can staring at a screen permanently damage my eyes?
There is no good evidence that ordinary screen use causes permanent damage to the eye in adults. It causes fatigue and dryness, which are uncomfortable but reversible. The exception is children, where more time on near work and less time outdoors is linked to myopia developing and progressing.
Do blue-light blocking glasses help with eye strain?
Clinical trials have generally not found a meaningful benefit for eye strain. Blue light does shift sleep timing, so reducing screen brightness in the evening is worthwhile, but blinking, focusing effort and desk setup are the far larger factors during the day.
How often should I have an eye test if I work at a computer?
Every two years for a healthy adult with no symptoms, and annually if you wear a prescription, are over 40, have diabetes, or have a family history of glaucoma. Come sooner if symptoms persist after two weeks of changing your habits.
Are over-the-counter eye drops safe to use every day?
Preservative-free artificial tears are safe used several times a day. Avoid drops marketed for redness for routine use — they work by constricting blood vessels and the redness returns worse when you stop. Any drop containing a steroid should only be used under supervision.
